Position Details

Under the direction of the agency’s clinical supervisor, the case manager utilizes their case work knowledge and skills to provide motivational interviews, assessment, care planning, advocacy and referral services to individuals with severe and persistent mental illness, living in a congregate residential setting.

Job Tasks include:

  • Conduct intake screening and assessments.

  • Provide access and referral for concrete services on behalf of residents.

  • Participate in staff rounds, clinical meetings and recreational community activities.

  • Maintain a professional network of other community resources and programs.

  • Conduct structured therapeutic activity groups.

  • Accompany resident on emergency medical or clinic visits.

  • Availability on evening and weekends for consultation as needed.

  • Maintain appropriate documentation and clinical notes in a timely manner.

Professional requirements::

  • Bachelor’s or master’s degree in Social Work or related field.

  • One year experience working with behaviorally challenged or homeless population in shelter setting.

  • Critical thinking skills.

  • Excellent verbal and written communication skills.

  • Computer proficiency.

  • Bi-lingual preferred.
